Tips for Applying SDOH Codes as Part of the Pursuit of Better Outcomes

Here are two CMS charts about the use of social determinants of health (SDOH) codes.[1] As CMS continues to bake health equity into the health care delivery system through Medicare reimbursement and pay-for-performance models, SDOH will play a more prominent role.[2]
