Form for Sequestering, Documenting Evidence During an Investigation

This form was developed by Moffitt Cancer Center in Tampa, Florida. It’s used when the compliance team is sequestering evidence, such as paper files and laptops, for an investigation, “to document who has laid hands on it at any given time and any hand off is appropriately documented,” said Donnetta Horseman, chief compliance officer at Moffitt (see story, p. 1).[1]  “It is to ensure the integrity of the investigation.” Contact her at donnetta.horseman@moffitt.org.
